【摘要】探讨慢性鼻-鼻窦炎患者睡眠障碍现状及影响因素。方法 选择2019年4月-2021年4月我院耳鼻喉科门诊80例慢性鼻-鼻窦炎患者为研究对象,采用匹兹堡睡眠质量指数量表(PSQI)调查患者睡眠障碍发生情况,并设为睡眠障碍组以及正常组。单因素分析两组患者性别、年龄、合并抑郁、鼻黏膜症状等资料,将差异具有统计学意义的项目纳入Logistic回归分析进行多因素分析。结果 80例慢性鼻-鼻窦炎患者检测出睡眠障碍28例,发生率35.00%;睡眠障碍组以及正常组合并焦虑、合并抑郁、合并阻塞性睡眠呼吸暂停(OSA)、鼻黏膜症状程度、年龄、学历、病程等资料差异具有统计学意义(P<0.05);两组性别、居住地差异无统计学意义(P>0.05);多因素分析显示,合并焦虑、合并抑郁、合并OSA、严重鼻黏膜症状是慢性鼻-鼻窦炎患者睡眠障碍的影响因素。结论 慢性鼻-鼻窦炎患者易发生睡眠障碍,受到负性情绪、病情严重程度、合并OSA等因素的影响,可采用针对性的心理干预及治疗改善睡眠质量。
[Abstract] Objective To explore the current status and influencing factors of sleep disorders in patients with chronic rhinosinusitis. Methods Eighty patients with chronic rhinosinusitis in our hospitals otolaryngology outpatient department from April 2019 to April 2021were selected as the research objects. The Pittsburgh Sleep Quality Index (PSQI) was used to investigate the occurrence of sleep disorders in patients, and set as sleep disorder group and normal group. Single factor analysis of the two groups of patients gender, age, combined depression, nasal mucosal symptoms and other data. The items with statistically significant differences were included in Logistic regression analysis for multivariate analysis. Results In 80 patients with chronic rhinosinusitis, 28 cases of sleep disturbance were detected, with an incidence rate of 35.00%. There were statistically significant differences in sleep disorder group and normal combination with anxiety, depression, obstructive sleep apnea (OSA), nasal mucosal symptoms, age, education, and course of disease (P<0.05). There was no statistically significant difference between the two groups in gender, residence, whether to keep pets, and whether to plant flowers and plants (P>0.05). Multivariate analysis showed that combined anxiety, combined depression, combined OSA, and severe nasal mucosal symptoms were the influencing factors of sleep disorders in patients with chronic rhinosinusitis. Conclusion Patients with chronic rhinosinusitis are prone to sleep disorders, and are affected by negative emotions, severity of the disease, and OSA. Targeted psychological interventions and treatments can be used to improve sleep quality.
